hmm I think I might know a possible answer to this conundrum, but I can't say for sure without knowing more about your situation.
When you download content from the xbox live marketplace it registers that content to the Xbox console that downloaded it (*Not hard drive, the xbox itself.) and to the gamer tag that downloaded it originally. This means that addintional profiles on an xbox with say a map pack for CoD4 would have acces to that map pack. However if you were given a new xbox instead of having your previous one repaired (They replace consoles quite frequently rather than repairing them.) Only the account that downloaded them, would have acces.
EX: (For this example we will lable them GT1 (Gamertag 1) GT2, C1 (Console 1) and C2.
GT1 downloads DLC on C1 --> GT2 has acces to DLC on C1
Replace C1 with C2, Hardrive stays the same.
GT1 continues to use DLC on C2 --> GT2 has no access to DLC on C2
