I think it happens like you get knocked up and a cutscene happens and you have an infant. When my male character got a girl knocked up it went to a cutscene about it and some game time passed. Time passes again later and the kid gets older. If you don't visit and support them bad things can happen. You can have more than one marriage and many more kids btw. It's a cool game
Oh and it also allows for gay stuff. My character is so awesome I have gay guys asking to marry me all the time. It's funny but it gets old after awhile.
